Terms and conditions for OPEX 4 Week Experience
__________________________________ ( print name ) understand that the term of this exclusive coaching contract is for 30 days beginning on the date this exclusive coaching contract is initialed as provided below and at the end of the 30 days term this exclusive coaching contract will NOT be renewed after the 30 day period. A New Contract will be signed. NO REFUND will be given due to any circumstances unless agreed upon by Genetic Potential Athletics LLC DBA OPEX STL Central.
Coaching Rate __________349.00__________ for 30 days.
Payments. Payment of the exclusive coaching dues will be automatically withdrawn from clients designated account every month on the first day of that month if the designated account does not have sufficient funds for the payment of the exclusive coaching dues or Client does not make payment of the exclusive coaching dues on or before the Third of the month then Genetic Athletics shall have the right to immediately discontinue the exclusive coaching programming and consultation services to the client until payment is received. Once the payment has been received the exclusive coaching programming and consultation services to the client will be continued as normal.
EFT request an authorization. Client hereby authorizes OPEX STLC to make periodic changes or withdraw from the account designated by client to make initial payment described above the designated account should be used for the payments and for any and all exclusive coaching dues fees late charges cost expenses or any other monies due to OPEX STLC under the terms and conditions of the exclusive coaching contracts. Client understands that client is entitled to notice of overage charges and withdraws under the EFT, but client waive the right to receive prior notice for charges which draws me with respect to any uncollected payments or portions of the balance due described above and the corresponding service charges both of which client agrees or not very charges or withdraw. Client may designate the replacement account here in upon 10 days written notice to OPEX STLC and approved by OPEX STLC of the replacement account.
